What to Expect

When you contact us, we'll help you:

• Schedule your initial consultation
• Complete necessary intake forms

New Patient Information

For new patients, we recommend:

• Calling or texting during business hours to schedule your first appointment
• Gathering any past psychiatric and medical treatment records
• Preparing any questions you'd like to discuss with your provider
• Completing all intake forms via the patient portal prior to your first appointment
• Arriving 15 minutes early for your first appointment

Insurance and Payment

We are currently in network with:

• Florida Blue (Lucet)
• Aetna
• Optum / United Behavioral Health

Please contact your insurance provider to verify your mental health coverage and discuss payment options. We also accept HSA/FSA forms of payment.

You are also welcome to pay out of pocket in a fee for service model. A superbill with CPT codes will be provided upon request if you would like to submit for reimbursement to your insurance at out of network rates.

Cash rates:

• Initial Visit: $300
• Follow Up Visits: $150 - $200 depended on time and service provided
